§ 1 Basic information about the Website

This Privacy Policy sets out the rights and obligations of the Users of the kacdoktor.pl website

§ 2 Glossary of terms

For the purposes of these Regulations of the kacdoktor.pl website operation:

1) Personal Data - means personal data, including special categories of personal data, constituting a set of information about a natural person that allows him to be identified;
2) Contact Form - means the form in which the Website Users provide the necessary data in order to receive information about the subject of activity and services provided by the Operator;
3) Operator - means KACDOKTOR SPÓŁKA Z OGRANICZONA ODPOWIEDZIALNOSCIA with its registered office in Warsaw, at the address: ul. Krochmalna 32a, 00-864 Warszawa, entered into the Register of Entrepreneurs of the National Court Register under KRS number: 0000744112, REGON: 380968218, NIP: 1132980622, with share capital of PLN 10,000 (say: ten thousand zlotys);
4) Regulations - means the Regulations for the operation of the kacdoktor.pl website, which define the rights and obligations of the Operator as well as the rights and obligations of the Website Users;
5) Website - means the website operating at www.kacdoktor.pl;
6) User - means any natural person visiting the Website and using its resources via the Internet.

§ 3 Privacy Policy

1. The Website's privacy policy is a document that concerns the protection and processing of Users' Personal Data, including specific categories of Personal Data. The Policy defines the rules for ensuring the privacy of Users based on the following legal provisions:
- 1) Regulation of the European Parliament and of the Council 2016/679 of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46 / EC (general regulation on the protection data, GDPR);
- 2) the Act of May 10, 2018 on the protection of personal data (Journal of Laws, item 1000);
- 3) the Act of 18 July 2002 on the provision of electronic services (i.e. Journal of Laws of 2019, item 123);
- 4) Act of 16 July 2004 Telecommunications Law (i.e. of 2018, item 1954 as amended).
2. The use of the Website is carried out via a secure connection. Information sent and received by the Website is private.

§ 4 Viewing the Website's resources by the User

The storage of server inquiries does not require the processing of Personal Data, because the data is not associated with specific Users browsing the Website pages. The resources viewed by the User are identified by URL addresses and relate to:

1) the public IP address of the end device from which the request came;
2) names of the User's station - identification performed by the http protocol, if possible;
3) User name provided in the authorization process;
4) time of the query;
5) the first line of the http request;
6) http response code;
7) the number of data sent by the server;
8) URL of the page previously visited by the User (so-called referrer link) - in the case when the Website was accessed via an external link;
9) information about the User's browser;
10) information about errors that occurred during the execution of the http transaction.

§ 5 Cookies Policy

1. The website uses the "cookies" technique. A cookie (also known as a "cookie") is a small file with information saved by the Website on the User's end device in connection with the User's use of the Website.
2. The use of "cookies" techniques does not allow the collection of any Personal Data and address of the User, or any confidential information from his computer. The "cookies" technique allows only to define the profile of the information the User is interested in. In this way, the collected data makes it possible to study the aggregated traffic of Users on the Website and for statistical purposes.
3. The purpose of using the "cookies" technique is to ensure the proper functioning of the Website, collecting anonymous statistics helping to determine how the content posted on the Website is used and presenting Users with information and advertisements tailored to their preferences.

6. As part of the Website, we also use Google Website Call Conversion (gwcc) cookies to redirect a telephone call from the Website, where the telephone number is indicated, to the mobile device from which the telephone call is made.

§ 6 Protection of Personal Data

1. The administrator of personal data of the Website Users is KACDOKTOR SPÓŁKA Z OGRANICZONA ODPOWIEDZIALNOSCIA with its registered office in Warsaw, at the address: ul. Krochmalna 32a, 00-864 Warsaw, entered into the Register of Entrepreneurs of the National Court Register under the KRS number: 0000744112, registered office of issuing REGON: 380968218, NIP: 1132980622.
2. Using the Website, no personal data of Website Users are processed, except for the personal data of Website Users who submitted a complaint in accordance with the Website Regulations or sent an inquiry in accordance with this Privacy Policy.
3. When downloading the Website User's telephone number via the Website, no personal data of the Website User is collected, and the downloaded Website User's telephone number is not processed using the Website or compiled with other personal data of natural persons using the services of KACDOKTOR SPÓŁKA Z OGRANICZONA ODPOWIEDZIALNOSCIA with its registered office in Warsaw .
4. The personal data of the Website Users are processed only for the purpose of settling the complaint and are processed as long as it is necessary
5. The Operator processes the User's Personal Data pursuant to art. 6 sec. 1 lit. b and f of GDPR.
6. The Website User, in connection with the processing of his Personal Data provided in the Contact Form, has the right to access the data, the right to rectify the data, the right to delete data, the right to limit data processing, the right to object to data processing and the right to request data transfer.
7. The Website User has the right to lodge a complaint with the supervisory authority - the President of the Personal Data Protection Office.
8. The Operator does not transfer the Personal Data of the Website Users to any third parties, in particular, personal data is not transferred to third countries outside the European Economic Area.
9. Personal data of all Website Users may be entrusted to third parties in order to use the services of an e-mail provider, provide legal services and use the IT website.
10. The personal data of the Website Users may be made available in accordance with applicable law at the request of law enforcement authorities, other public administration bodies, the Police and other services. The provision of personal data does not require the consent of the Website User.

§ 7 Final provisions

1 The operator reserves the right to amend this Website Privacy Policy in order to ensure that it is up-to-date and complies with applicable law. The change of the Website's Privacy Policy may also result from changes in technology, methods, purposes or legal grounds of the Users' personal data being processed.
